
We are committed to improving heart failure services so that all local people can access high quality care no matter where they live.
As a result, we are developing plans to improve services, building on what already works well and creating a more consistent and effective approach to heart failure care across the whole of Sussex.
However, we do not want to make any changes to services without involving you, our service users. We are keen to understand your experiences and gain your views on what a good service would look like for you.
We have launched a short survey for people in Sussex who live with heart failure or are a carer or family member of someone who does. Your responses will help us understand what local people think of existing services and give us ideas about how services could be improved.
